Physician Quality Reporting System (PQRS)

Until recently, physician practices that wanted to participate in CMS’ Physician Quality Reporting System (PQRS) had to submit claims or registry data. Now they can report these data directly from their electronic health record. DFMC can provide physician practices on-site or telephone consultation to help accurately capture the required PQRS data elements and extract them for reporting purposes. This qualifies practices for incentive payments in addition to their annual Medicare reimbursement payments. Learn more about local improvement initiatives to leverage electronic health record functionality.

Physician offices also play a vital role in reducing avoidable hospital readmissions. Recent studies show that more than 15% of Medicare beneficiaries are readmitted within 30 days of discharge at an estimated cost of $12 billion annually. Interventions to improve handoffs between hospitals, physician practices, nursing homes, and others along the continuum of care are well documented. DFMC is working on community-based care transitions programs around the state. Learn more about this initiative.
