Core Prevention Overview
The overall goal of the Core Prevention theme under the Centers for Medicare & Medicaid Services' (CMS') 9th Scope of Work (SoW) is to improve the quality and frequency of preventive health care services in order to optimize Medicare beneficiaries’ quality of life.
Delmarva is working with participating physician practices to increase screening rates for breast cancer and colorectal cancer, and increase immunization rates for influenza and pneumococcal pneumonia vaccines.
Delmarva has recruited participating providers using electronic health records (EHR) certified by the Certification Commission for Healthcare Information Technology (CCHIT)to assist with collection of baseline data for the Core Prevention Theme. The measures include:
- Breast Cancer Screening for women between 52 or 42 and 69 years of age
- Colorectal Cancer Screening for beneficiaries between 51 and 80 years of age or 75
- Influenza Immunization for beneficiaries age 65 or older
- Pneumococcal pneumonia Immunization for beneficiaries age 65 or older
Delmarva is eager to work with the participating physicians to ensure beneficiaries receive the preventive screenings necessary to maintain a healthy life while assisting physician practices to reach the next level of best practice.
