Every Diabetic Counts!
Every Diabetic Counts is a free, diabetes self-management education program designed specially for underserved* Medicare beneficiaries with diabetes. Delmarva Foundation for Medical Care is partnering with physician practices and local community organizations to bring Every Diabetic Counts to eligible Medicare beneficiaries in Baltimore City, Baltimore County, and Prince George’s County.
Every Diabetic Counts classes are taught by experienced health educators and are held at easy-to-reach locations, including senior centers, senior apartment buildings, churches, and public libraries.
Every Diabetic Counts classes cover a variety of important topics, including:
- Managing blood sugar, blood pressure, and cholesterol
- Making healthy lifestyle choices, including diet and exercise
- Understanding medicines for diabetes
- Reducing medical complications
Classes provide practical, hands-on activities; lively discussions; helpful handouts; and many other tools to help patients improve their diabetes care. Studies show that diabetes self-management education (DSME) delivers culturally appropriate educational material that increases patients’ knowledge, skills, and confidence in managing their condition. DSME patients become “activated,” which leads to improved clinical outcomes. In addition, DSME programs like Every Diabetic Counts are recommended by the American Diabetes Association and the American Association of Diabetes Educators.
