DFMC is part of a network of Medicare quality improvement organizations—one in each US state and territory—committed to improving health quality at the community or local level. DFMC works with patients; providers and practitioners; and other stakeholders across organizational, cultural, and geographic boundaries to spread best healthcare processes and facilitate rapid, large-scale change.
DFMC drives change by bringing together local communities to problem-solve, share, and create solutions for improving the delivery of care called learning and action networks (LANs) or learning circles. Participants can harness the power of a 24/7 community to address common challenges, connect with peers for mentoring, and be among the first to know about improvement breakthroughs—and how they can replicate them in their own facility.
DFMC invites you to join LANs and Learning Circles focused on the following improvement initiatives:
- Reducing central line acquired blood stream infections (CLABSI)
- Reducing catheter-associated urinary tract infections (CAUTI)
- Care transitions
- Eliminating the use of physical restraints
- Reducing pressure ulcers
- Reducing adverse drug events
- Physician Quality Reporting System
- Cardiac Health
