DFMC is part of a network of Medicare quality improvement organizations—one in each US state and territory—committed to improving health quality at the community or local level. DFMC works with patients; providers and practitioners; and other stakeholders across organizational, cultural, and geographic boundaries to spread best healthcare processes and facilitate rapid, large-scale change.
DFMC drives change by bringing together local communities to problem-solve, share, and create solutions for improving the delivery of care called learning and action networks (LANs) or learning circles. Participants can harness the power of a 24/7 community to address common challenges, connect with peers for mentoring, and be among the first to know about improvement breakthroughs—and how they can replicate them in their own facility.
